- End to end client services management: intake, service (and risk) assessment, reviews, service transfer and coordination of direct support activities and understanding customer’s monthly statement.
- Working with customers, their supports and other providers to achieve their goals in line with their individual plans.
- Maintain accurate and up to date case notes on customer progress.
- Maintain an up-to-date knowledge of local services and pathways that customers can access and current sector systems and practices.
- Facilitate care worker support and supervision including accurate documentation and record keeping.
- Maintain internal filing, including accurate client documentation and other miscellaneous filing.
- Complying with all relevant legislation.
- Coordinating services to match client’s needs
- Clean the bed and mattress, bedroom according to the schedule (agreement with the family), ensuring that it is always clean and ventilated.
- Laundry, washing, drying providing auxiliary work for patients as required.
- Showering, dressing, grooming and providing relaxation massage to the client.
- Providing client’s mouth hygiene.
- Monitoring and caring to ensure the client is always in safe condition.
- Preparing meals necessary for the day, suitable for each individual client (if required).
- Psychological caring to client. Taking client to visit friends, doctors, hospitals
- Monitor and following the advice of nurses and Doctors to help clients staying in healthy life being.
