SWEP Funding Scheme

The State-wide Equipment Program (SWEP) is a sub-division of Ballarat Health Services which oversees a number of different ‘assistive technology’ programs.

SWEP funding provides Victorian people who either have a permanent or long-term disability or are frail aged with subsidised aids, equipment and home and vehicle modifications to enhance their independence and facilitate community participation.

SWEP gives you greater choice and flexibility in how you purchase your products from a supplier of your choice.

How can SWEP funding help you?

SWEP provides a subsidy towards the cost of equipment and/or modifications. This is to improve independence in your home, assist in community participation and support families and carers in their role. Subsidies and reissued equipment are provided through a number of programs, details of which are available below. Reissue equipment will be provided where appropriate.

How do I apply for SWEP Funding?

A SWEP funded order is processed as follows:

1) Print and compile the Eligibility Form
2) Bring this form to your local doctor or healthcare prescriber
3) The prescriber will assess your eligibility, process the form, and send it to SWEP
4) SWEP will raise a Purchase Order and send this to Independence Australia

You need to be assessed by an appropriate health professional from your local area who will help you determine the most suitable equipment or modification needed. This includes an assessment of whether you or your carer can safely use the equipment.

If you are a new client to SWEP applying for a VAEP or CEP program you may need to complete the Eligibility Form with all your personal details. If so, you will need to make an appointment to see your treating doctor/specialist to complete the last page of the Eligibility Form on your behalf. This is to confirm that you are eligible to receive assistance from SWEP.

How do I buy SWEP products?

Independence Australia is an approved supplier for SWEP. The new SWEP product list will be available online shortly.
